چکیده

Better model to predict arrhythmia recurrence post pulmonary vein isolation (PVI) for persistent atrial fibrillation (AF) is needed. Atrial epicardial adipose tissue (aEAT) and coronary arteriosclerosis has been reported associate with left substrate of AF. We aimed assess the influence aEAT artery calcium score (CACS) on outcome PVI evaluated consecutive 105 patients (age 62±10 years, 86 males) who underwent computed tomography (CT) prior initial quantified volume CACS using CT, defined three categories CACS: low;0, intermediate; 0-100, high; >100. The primary endpoint was any arrhythmias. During mean follow-up 282±183 days, occurred in 29 / (28%) patients. In univariable cox regression analysis, low high CACS, larger adjusted longer AF duration were associated recurrence. multivariable (hazard ratio [HR] 6.2, 95% confidence interval [CI]; 1.45-26.3, p=0.014, compared intermediate CACS) (HR; 3.7, CI; 1.26-10.7, p=0.017) independently PVI. Adjusted younger age (p=0.03), male (p=0.015), higher body mass index (p<0.001), diabetes mellitus (p=0.002). older (p<0001), (p=0.018) voltage area (≦0.5mV) (p=0.038). receiver-operating characteristics curve showed that improved predictive value beyond (area under curve: 0.72 0.53, respectively, p=0.010). Low Assessment pre-procedural CT may be useful determine ablation strategy

Pulmonary vein isolation for paroxysmal and persistent atrial fibrillation.

BACKGROUND The pulmonary veins (PVs) have been demonstrated to often play an important role in generating atrial fibrillation (AF). The purpose of this study was to determine the safety and efficacy of segmental PV isolation in patients with paroxysmal or persistent AF. METHODS AND RESULTS In 70 consecutive patients (mean age, 53 +/- 11 years) with paroxysmal (58) or persistent (12) AF, segme...

Epicardial adipose tissue and atrial fibrillation.

Atrial fibrillation (AF) is the most frequent cardiac arrhythmia in clinical practice. AF is often associated with profound functional and structural alterations of the atrial myocardium that compose its substrate. Recently, a relationship between the thickness of epicardial adipose tissue (EAT) and the incidence and severity of AF has been reported. Adipose tissue is a biologically active orga...

Pulmonary vein isolation for atrial fibrillation.

Nonpharmacological treatment for atrial fibrillation (AF) has evolved dramatically over the past 20 years. Pulmonary vein isolation, a catheter ablation technique developed in the last decade, prevents focal triggers in the pulmonary veins from initiating episodes of AF. Although the procedure initially involved focal ablation with a catheter directly in the pulmonary veins, investigators subse...

Atrial ectopy predicts late recurrence of atrial fibrillation after pulmonary vein isolation.

BACKGROUND Late recurrence of atrial fibrillation (AF) after radiofrequency ablation remains significant. Asymptomatic recurrence poses a difficult clinical problem as it is associated with an equally increased risk of stroke and death compared with symptomatic AF events. Meta-analyses reveal that no single preablation patient characteristic efficiently predicts these AF recurrences. This study...
